In time, gravity, sun exposure, and the stresses of daily life take their toll on our faces: deep creases appear beside the mouth, the jaw line slackens, and folds and fat deposits appear on the neck. A facelift is designed to smooth the sagging skin on your face and neck, tighten the underlying tissues, remove excess fatty tissue, and refine your neckline. The incisions are hidden within your hair and in the natural creases of your skin.

A facelift can be combined with fat injection placed strategically to give a softer look. It can also be combined with a browlift, eyelid surgery or nose reshaping for more dramatic results, or it can be restricted to the neck (necklift) if the patient's problems center there.

Our goal is to refresh and rejuvenate your facial appearance, make you appear younger, but maintain a very natural appearance.

The side effects from the facelift include temporary discomfort, swelling, bruising and numbness. In general, the swelling and bruising will begin to resolve within the first week and you will be able to start wearing makeup. Most of our patients resume their normal activities within two weeks. Full recovery from the bruising and swelling may take up to 3 weeks.

Forehead Lift

Browlifts can revitalize drooping or lined foreheads, helping you to look less angry, sad or tired. Using either traditional or minimally invasive methods, the muscles above the eyes are altered to smooth lines and raise the eyebrows for a more youthful appearance. The procedure is often combined with other operations such as blepharoplasty and facelifts to improve the look of the eyes and other areas of the face. The side effects from a forehead lift include temporary discomfort, swelling, bruising around the eyes and forehead numbness. In general, the swelling and bruising will begin to resolve within the first week and you will be able to start wearing makeup. Most of our patients resume their normal activities within two weeks. Full recovery from the bruising and swelling may take up to 3 weeks.

Eyelid Lift (blepharoplasty)

Your eyes are the focal point of your face. Cosmetic eyelid surgery is designed to correct puffy bags below the eyes and wrinkled, drooping upper eyelid skin that can give you a tired, sad, or angry appearance. The incisions are hidden within the natural creases of your upper and lower eyelids so they will heal inconspicuously. This surgery is designed to brighten your entire face and minimize the effects of aging.

The side effects of this surgery include temporary discomfort, tightness of the lids, swelling, and bruising. Most of our patients resume their normal activities within one week. Full recovery from the bruising and swelling may take 2 to 3 weeks. Patients may resume wearing contact lenses after 2 weeks.

Facial Fat Grafting

Fat injections can be an effective way to reduce wrinkles, diminish acne scarring and regain a more youthful appearance. By injecting human fat harvested from your own body, we can enhance facial fullness, fill deep creases, soften facial creases and wrinkles, plump lips and build up shallow contours.

Fat injection is a more extensive procedure than the use of other cosmetic injectable fillers because liposuction techniques are involved. These techniques are used to extract the fat prior to injection. Fat injection is either performed alone or in conjunction with a facelift to enhance and soften facelift results.

Facial Sculpting

Facial liposuction specifically the neck/chin, is a surgical procedure that has been used in the United States for almost 20 years to remove localized collections of fatty tissues. Many people have isolated areas of excess fatty tissue that can cause the facial form to lose definition. Facial Liposuction can be used safely and effectively to improve the contour of the face, especially the neckline. The entire procedure is performed through inconspicuous incisions.

Good candidates for facial sculpting are patients who have excess fatty tissue without a lot of excess of skin. The procedure is most effective in patients that have healthy, elastic skin. Patients who have both excess fatty tissue and a lot of excess skin will have the best result from a combined facial sculpting and face-lift procedure. Side effects from the surgery include temporary bruising, swelling, and numbness.

Facial Implants

Chin implants reshape the chin to help provide a harmonious balance to the facial features. Many times a plastic surgeon may recommend chin enhancement to a patient having nose surgery in order to achieve a more satisfying facial proportion, since the size of the chin may magnify or minimize the perceived size of the nose and vice versa. Whether or not we notice it consciously, full cheeks are often a part of how we judge a person's face. Cheek implants can have a major impact on the appearance and self-confidence of patients whose cheeks have thinned or sunken due to age, illness or other causes. Implant sizes and shapes are tailored to each patient's desired appearance. You will notice an improvement in your facial harmony immediately. You may experience soreness in the area of the implants, but this rapidly disappears. A very mild swelling usually takes several weeks to subside. You should be able to resume your normal daily activities the day after surgery, and you should be able to resume all of your physical activities within two weeks of surgery.

Cleft Lip and Palate

Cleft lip and cleft palate are facial and oral malformations in the baby when it is developing inside the mother’s womb. Clefts results when there is not enough tissue in the mouth or lip area, and when the tissue that is available does not unite together as it should.

A cleft lip is a physical split or separation of the two sides of the upper lip and appears as a narrow opening or gap in the skin of the upper lip. This separation often extends beyond the base of the nose and includes the bones of the upper jaw and/or upper gum.

A cleft palate is a split or opening in the roof of the mouth. A cleft palate can involve the hard palate (the bony front portion of the roof of the mouth), and/or the soft palate (the soft back portion of the roof of the mouth).

Cleft lip and cleft palate can occur on one or both sides of the mouth. Because the lip and the palate develop separately, it is possible to have a cleft lip without a cleft palate, a cleft palate without a cleft lip, or both together.

The problems associated with cleft lip and/or palates are: feeding problems, ear infections/hearing loss, speech problems and dental problems:
The Treatment for Cleft Lip and Cleft Palate: Cleft lip requires one or two surgeries depending on the extent of the repair needed. The initial surgery is usually performed by the time a baby is 3 months old. Repair of a cleft palate often requires multiple surgeries over the course of 18 years. The first surgery to repair the palate usually occurs when the baby is between 6 and 12 months . The initial surgery creates a functional palate, reduces the chances that fluid will develop in the middle ears, and aids in the proper development of the teeth and facial bones.

Children with a cleft palate may also need a bone graft when they are about 8 years old to fill in the upper gum line so that it can support permanent teeth and stabilize the upper jaw. About 20% of children with a cleft palate require further surgeries to help improve their speech.

Once the permanent teeth grow in, braces are often needed to straighten the teeth.

Additional surgeries may be performed to improve the appearance of the lip and nose, close openings between the mouth and nose, help breathing, and stabilize and realign the jaw. Final repairs of the scars left by the initial surgery will probably not be performed until adolescence, when the facial structure is more fully developed.

What Is the Outlook for Children With Cleft Lip and/or Cleft Palate?
Although treatment for a cleft lip and/or cleft palate may extend over several years and require several surgeries depending upon the involvement, most children affected by this condition can achieve normal appearance, speech, and eating.

Ear Surgery (Otoplasty)

Ear reshaping, also known as otoplasty or ear pinning, can make your ears less prominent or make them appear more symmetrical. For unknown reasons, one or both ears of some individuals protrude further than normal from the head. This is usually a result of the failure of one of the folds of the ear cartilage to fully develop. The procedure can be performed in children or adults. Children typically undergo surgery just prior to entering school, to minimize teasing by schoolmates.

An incision is made over the back of the ear, so that the scar is virtually invisible. The cartilage is then reshaped and sutured into its normal position.

Afterwards, the incisions are carefully closed to minimize the scar, and a sterile dressing is applied to the incisions. You will notice an improvement in the shape of your ear(s) immediately. A very mild swelling usually takes several weeks to subside. You should be able to resume your normal daily activities the day after surgery, and you should be able to resume all of your physical activities within two weeks of surgery.

Botulinum Toxin

Botulinum Toxin is a purified natural protein that relaxes facial muscles by blocking nerve impulses. Relaxing specific muscles in the face through injections of the Botulinum Toxin formula reduces facial lines caused by frowning and creates smoother skin, fewer wrinkles, and a more relaxed appearance. Botulinum Toxin works best in the upper third of the face: frown lines between the eyebrows, horizontal bands of wrinkles across the forehead, and crow’s feet at the corners of the eye. Botulinum Toxin can also be used to soften or erase lines around the mouth, to sculpt the face by raising one or both eyebrows, as well as to smooth vertical and horizontal neck lines by relaxing the platysmal (neck) muscles.

Botulinum Toxin may also be useful for migraine headaches, excessive sweating, and eye and neck muscle spasms.

No recovery time is needed. You can return to work or your daily activities right away. Effects are usually seen within a few days and reach maximum benefit by fourteen days following injection. It is important to remember that Botulinum Toxin is not a permanent solution for troubling wrinkles because the effects are not permanent. For many patients, however, the effects have been quite dramatic. Although individual results vary, the results achieved from a single treatment usually last three to six months. You will notice a gradual fading of the effects. To maintain the effects, treatments are usually required two to three times a year. Botulinum Toxin Cosmetic treatment may be repeated indefinitely. Currently Botulinum Toxin Cosmetic has been used safely for over twenty years worldwide. Acceptable safety in long-term treatment has been well established.

Dermal Fillers

As we age, our skin begins to lose its fullness, and sagging, wrinkling and hollowing occur. These changes and other effects of aging can be improved through the use of injectable fillers. These treatments, often made from collagen, hyaluronic acid gel and other natural substances, are injected directly into the skin to restore fullness and smooth out wrinkles and other fine lines. The injected substance supports the skin and restores the volume lost over time. Many of the specific treatments have been approved by the FDA and are considered safe procedures with little to no side effects.

Types of fillers commonly used:

Hyaluronic Acid (Restylane®, Juvederm®, Perlane®)

A naturally occurring protein found in human skin, hyaluronic acid has now replaced collagen as the current state of the art in soft tissue fillers. There is no risk of allergic reaction, and hyaluronic acid products typically last much longer than collagen before they are harmlessly reabsorbed by the body. However, it gives long-lasting results because as some of the hyaluronic acid is absorbed, the remaining amount retains its volume by binding water. This allows hyaluronic acid to give long-lasting results that remain nearly constant for the entire period.

Radiesse® This dermal filler is made of a synthetic substance of calcium hydroxylapatite, but there have been no allergic reactions associated with it. Most patients can return to normal activities the next day. One of the advantages of Radiesse® is that it can last up to three years.

Dermal fillers are commonly used for:

• Facial folds and wrinkles: Dermal fillers are an excellent complement to Botulinum Toxin injections because they correct a different type of wrinkle.

• Restoring facial volume: As we age, our face naturally loses volume and what fat we have is often displaced by gravity. This gives the face a wan, tired appearance. Dermal fillers can be used to fill in areas that have lost volume, restoring a youthful fullness to your face.

• Lip augmentation: The lips also tend to lose volume as we age, which is especially important to women. Thin lips can create an older, sterner appearance. Dermal fillers are great treatment for restoring volume to lips.

• Facial reshaping: Some people do not have the facial shape they desire, and in many cases this can be at least partly corrected with dermal fillers, which can give the appearance of higher cheekbones or a stronger chin. The only common side effect of injectable fillers is swelling at the injection sites which can last 1-3 days, and is not something that everyone experiences. You may be aware of the filler at first, but this will pass quickly. As soon as the swelling goes down, the results of your treatment will be apparent. Your wrinkles, folds, and hollows will be significantly reduced. They may even be erased. Your results should last for about six months or more depending on the type of filler used and its placement up- to two years.

PRP (Platelet Rich Plasma)

Platelet Rich Plasma (PRP) is a by-product of blood (plasma) that is rich in platelets. Platelet Rich Plasma (PRP) contains various growth factors that can be used to enhance healing and provide a matrix to stimulate collagen deposition for volume enhancement and potential improvement in skin quality and contour. PRP has a very high concentration of growth factors that help stimulate skin rejuvenation and restore volume due to atrophy and aging.

During the office procedure a small amount of your own blood is drawn out via the IV. This blood is then placed in the PRP centrifuge machine and spun down. In less than fifteen minutes, the PRP is formed and ready to use to be injected into the skin for rejuvenation after a topical anesthetic is placed on the skin. When PRP is injected into the skin, it accelerates the normal healing process by stimulating collagen formation, revascularization and repair.

This process brings long lasting rejuvenation without side effects. Results usually become apparent within 15 days of treatment.

Its advantages are that it promotes local tissue growth and repair, stimulates regeneration to eliminate wrinkles, is safe since it is made from one’s own blood and is an office procedure.

PRP is recommended for any age groups at different frequencies:

Age 30 – 40 years: In this age category the main aim is rejuvenation and prevention of collagen degradation and re- stimulation of new collagen and elastin production, thus attempting to slow down the ageing process. One treatment every 2 years is sufficient.

Age 40 – 50 years: In this age category the aim is rejuvenation and to attempt reversal of the ageing process which is more difficult. More re-stimulation of collagen and elastin production is necessary. The initial treatment should be followed by a second treatment 2-6 months later and a third treatment 9 months after the second treatment. Follow-up ‘maintenance’ treatments may be necessary every 12-18 months.

Age 30 – 40 years: In this age category the main aim is rejuvenation and prevention of collagen degradation and re- stimulation of new collagen and elastin production, thus attempting to slow down the ageing process. One treatment every 2 years is sufficient.

Age 50+ years: In this age category the aim is rejuvenation and to attempt more intense reversal of the ageing process. The initial treatment should be followed by a second treatment 2-6 months later and a third treatment 6 months after the second treatment. Follow-up ‘maintenance’ treatments may be necessary every 12 months

Thermage® Skin Tightening

Thermage® is a safe and effective procedure that stimulates your skin to produce more and better collagen, which makes it tighter and healthier-looking. Collagen is the major building block of the skin. This protein helps the skin to be tight and elastic, meaning that it returns to its shape after pulled or otherwise deformed. Collagen-rich skin resists wrinkling, is thicker, and appears more youthful. Thermage® uses a unique form of radio frequency (RF) energy to simultaneously tighten existing collagen and stimulate the skin to produce more collagen. The result is tightened skin producing a kind of “lift” without a single incision. With minimal treatment discomfort and no recovery, tighter, smoother skin, an overall more youthful appearance can be achieved. The energy is delivered through a hand-piece directly applied to the skin heating the deeper layers causing tightness and simultaneously cooling the skin for comfort. The addition of the new CPT hand-piece allows the heat to be more evenly distributed across the treatment area, permitting the Thermage® CPT system to heat four times more tissue to therapeutic temperatures than rival technologies and thus significantly increasing its effectiveness.

Thermage® is effective on:

The Eyes – treating upper and lower eyelids to reduce hooding and fine lines, tightening creppy skin as well as lifting of the brow line

The Face – smoothing and tightening sagging skin, smoothing fine lines and wrinkles and helping define the jawline and lip. Addresses loose jowls and “turkey neck”

The Body – helping contour loose skin and smooth bulges. Treating wrinkles, creppy or sagging skin on the tummy, knees, arms, legs, hands or buttocks example after weight loss or pregnancy. Effective on cellulite.

After treatment the skin may be slightly red, but most people resume regular activities immediately after treatment. There is no recovery time or special follow-up care. The results may appear immediately but usually, measurable tightening and contouring appear gradually in the 2 to 6 months post treatment. Results can last years depending on the skin's condition and ageing process.

While Thermage® cannot produce the same level of effects as a surgical facelift, it often produces visible results that for many people are well worth the tradeoff of a non-surgical procedure with no recovery.

Velashape III

VelaShape® is a non-invasive body contouring treatment for circumferential and cellulite reduction*. VelaShape enables you to safely achieve a toned, contoured and well shaped body in typcially 3 treatment sessions (single treatment protocol is also available); making you look and feel more youthful. VelaShape provides dramatic results without downtime or significant discomfort.

The elōs technology combination of infrared light, coupled radio frequency energy rollers and vacuum causes deep heating of the fat cells, their surrounding connective tissue and the underlying dermal collagen fibers. This type of efficient heating and vacuum stimulate the growth of new and better collagen and elastin which results in localized reduction in skin laxity, body volume and an overall improvement in skin structure and texture.

Velashape III helps to remove excess body fat and treating cellulite from problem areas such as the abdomen, thighs, arms, buttocks. Muffin top and chin.

Treatment is safe and effective for all skin types and colors. There are no reported short or long-term health effects.

Most patients find VelaShape III comfortable – like a warm deep tissue massage. The treatment is designed to accommodate your sensitivity and comfort level

VelaShape III is a no-downtime treatment, though it’s normal to experience a warm sensation in the treatment area or notice that it looks a little pink for a few hours after the procedure. You should be able to return to your regular activities right away.

Fractional Radio Frequency Microneedles (RFM)

The combination of microneedles and radiofrequency leads to substantial improvement in skin quality and texture. The procedure involves penetration of microneedles into the dermis to deliver high-tensioned radiofrequency pulse into the targeted tissue. This results in the body’s own production of elastin and collagen fibres, making skin firmer and healthier.

The microneedles are driven directly into the skin at a certain depth, and radiofrequency energy is then released inside of the skin. This gives a more uniformed deep tissue heating that will stimulate new collagen fibers production. The needle tip contains 25 very small gold needles acting as an internal conductor. In cases of acne, the RF energy kills the bacteria responsible for the lesions and shrinks the sebaceous gland.

Fractional Radio Frequency Microneedles Treatment is effective for:

• Acne and acne scars

• Wrinkles & Fine lines

• Open pores

• Stretch marks and skin laxity

• Scars of all types

The treatment area is numbed for 30 minutes before starting. The procedure would take approximately 20-30 minutes to do depending on the area and pain threshold. Immediately after the procedure, the treated area is warm, red and swollen. After 2 days the needle marks will disappear.

After a few treatments you will see a noticeable difference, it will help both improve wrinkles and appearance of the skin by tightening, rejuvenating, reducing pore size and minimizing scars.

Chemical Skin Peels

Chemical peels have long been used as skin rejuvenation procedures. In the right hands, chemical peels can safely address many common facial concerns, including wrinkles, uneven skin coloration, and a dull complexion. Depending on the severity of the problem the peel is designed to address, you can often see results immediately with little or no recovery necessary.

A chemical peel involves the application of medical acids to remove the damaged skin layers in order to produce more youthful appearance. A healing response to the peel can also triggers new collagen formation so the skin appears tighter and more elastic.

Chemical peels are most commonly used on the face, but can be used on other body areas such as the chest, back, shoulders and arms to eliminate lines, freckles, or age spots. Patients with more extensive sun damage on the legs can also be treated with chemical peels.

Chemical peels have been found to be a powerful complement to other skin treatments, leading to numerous innovations in what is known as combination therapy.

Theoretically, chemical peels can be combined with any light, laser, or skin therapy.

The most popular combinations include:

• Thermage skin tightening non-surgical radiofrequency treatment to tighten and lift sagging skin
• Topical prescriptions that are effective against wrinkles like Retin A™
• Dermal fillers like Restylane, Juvederm, Perlane, Radiesse, to smooth the deeper lines
• Botulinum Toxin to relax muscles that contribute to wrinkles

We offer a range of chemical peels at AlShunnar Plastic Surgery that effectively treat sun damage, age spots, uneven pigmentation, acne scars and blemishes, fine lines, and wrinkles. Our highly trained and experienced professionals will analyze your skin and choose the appropriate peel for your skin type and personal needs. These peels are performed in our office and result in exfoliation of the outer layers of your facial skin. The healing time depends on the depth of the peel. As a rule, the deeper the peel, the longer the recovery time. In all cases, avoid sun exposure immediately after a chemical peel and use sun block during the healing process since the new skin is fragile and more susceptible to injury.

With superficial peels, you should be able to resume your normal daily activities immediately following your procedure, including the application of makeup and sunscreen. Within minutes to days, depending on the type of peel chosen, you will notice that the treated area is more smooth with less wrinkles, having a more youthful appearance. Your newly rejuvenated skin should have a healthy glow and appear soft and supple. With deeper peels, you will go through a process of shedding and healing that should be complete seven to ten days following your peel. At this time you will notice a marked improvement in the quality of your facial skin.

The Jan Marini Glycolic Peel
The Jan Marini peel is a glycolic acid peel system that provides controlled and predictable removal of surface skin cells, stimulating skin renewal to improve the appearance of the skin.

You will notice a difference in your skin texture immediately. The procedure is fast and convenient, taking only about fifteen minutes to half an hour. And with virtually no recovery time, you can be back to your normal routine the same day. There is no significant pain or discomfort or need for anaesthesia.

It takes usually 4-6 treatments at three to four week intervals to achieve the desired effect. All body areas appear to respond well to treatment with glycolic acid; however, the arms, hands, neck and chest all improve at a slower rate than the face. Some redness is temporary and normal and may be covered with makeup.

Melanin Control System® (MCS)

One of the major causes of skin photo-aging and pigmentation are caused by excessive sun exposure which stimulate melanocytes to produce excess melanin. MCS works nicely on all skin types to accelerate the turnover of skin cells, remove lesions, restore elasticity and helps to improve the true moisturizing characteristics of the skin. MCS is a professional treatment intended for treating aging skins or those which display pigmentation issues such as photo-ageing, chrono-ageing, melasma/chloasma, residual pigmentation and seborrhoeic skin. It stimulates the fibroblast and therefore the collagen and elastin that maintain the young and elastic skin. It is very effective in reducing age spots, freckles and dark pigmentation due to pregnancy or taking birth control pills (melasma or Chloasmas). It also treats wrinkles caused by sun damage, aging and hereditary factors in the face as well as body. In addition, it helps in the control of different types of acne and improves the look and feel of skin that is dull in texture and colour.

VI Peel
This peel reverses the effects of sun damage, environmental factors, and time, to enhance your skin’s natural beauty. The Vi Peel has been developed as a unique product which gives patients dramatic results without the pain and lengthy recovery time that comes with similar procedures. The Vi Peel does more than remove visibly damaged skin. It uses the synergy of all available and known modes of action of various acids to produce a multitude of positive effects. It tackles skin problems at the cellular level rather than just burning skin off. Thanks to the concerted actions of its many acids and Vitamins, it will help erase fine lines and wrinkles, shrink enlarged pores and build collagen and elastin which tighten skin. It is extremely effective in practically eliminating hyper pigmentation including melasma, the most difficult pigmentation issue to address. The VI Peel also provides excellent results for patients with acne. Its benefits also include that it is safe for all skin types and ethnicities, is the only peel safe to use on the eye area, neck, chest and hands, does not require preconditioning, is virtually discomfort free and there is little downtime. For an optimum result, the Vi Peel can be incorporated into a maintenance program and performed every six months.

To keep your skin its healthiest, it is best to get periodic deep cleanings. Deep cleanings of the skin involve removing the outer dead layers of skin to expose the fresher, living skin below. This is known as exfoliation and microdermabrasion is an excellent tool for this. Microdermabrasion is a good treatment for acne-prone individuals, those who desire a deep cleaning for healthy skin, and people with dull-looking skin. It removes skin by buffing it with a stream of fine crystals. Air pressure drives the crystals at the skin at a very high velocity, and the same pressure creates a vacuum that draws the crystals away. The process will remove a very thin layer of skin, less than a thousandth of an inch thick. This outer layer consists of dead skin that has lost the luster of living tissue and has suffered damage from the sun and other exposure. It also contains contaminants, pollutants, and dirt that clog pores and damage skin.

Microdermabrasion takes less than an hour, and because it is minimally invasive you can go back to work right away with your skin looking healthy and glowing, but not red or irritated. Acne and skin dullness can be reduced with microdermabrasion. Some people see a reduction in skin discoloration, fine wrinkling, and scaly skin after a regular treatment program. Some of the benefits of microdermabrasion are not visible, though. Microdermabrasion prepares your skin to receive topical nutrients, and combining the procedure with a recommended skin care regimen can deepen and lengthen the results you see from microdermabrasion.
