Dermal Fillers

As we age, our skin begins to lose its fullness, and sagging, wrinkling and hollowing occur. These changes and other effects of aging can be improved through the use of injectable fillers. These treatments, often made from collagen, hyaluronic acid gel and other natural substances, are injected directly into the skin to restore fullness and smooth out wrinkles and other fine lines. The injected substance supports the skin and restores the volume lost over time. Many of the specific treatments have been approved by the FDA and are considered safe procedures with little to no side effects.

 

Types of fillers commonly used:

 


Hyaluronic Acid (Restylane®, Juvederm®, Emervel and Teosyal)

A naturally occurring protein found in human skin, hyaluronic acid has now replaced collagen as the current state of the art in soft tissue fillers. There is no risk of allergic reaction, and hyaluronic acid products typically last much longer than collagen before they are harmlessly reabsorbed by the body. However, it gives long-lasting results because as some of the hyaluronic acid is absorbed, the remaining amount retains its volume by binding water. This allows hyaluronic acid to give long-lasting results that remain nearly constant for the entire period.

 

Radiesse® This dermal filler is made of a synthetic substance of calcium hydroxylapatite, but there have been no allergic reactions associated with it. Most patients can return to normal activities the next day. One of the advantages of Radiesse® is that it can last up to three years.

 


Dermal fillers are commonly used for:

 

• Facial folds and wrinkles : Dermal fillers are an excellent complement to Botulinum Toxin injections because they correct a different type of wrinkle.

 

• Restoring facial volume : As we age, our face naturally loses volume and what fat we have is often displaced by gravity. This gives the face a wan, tired appearance. Dermal fillers can be used to fill in areas that have lost volume, restoring a youthful fullness to your face.

 

• Lip augmentation : The lips also tend to lose volume as we age, which is especially important to women. Thin lips can create an older, sterner appearance. Dermal fillers are great treatment for restoring volume to lips.

 

• Facial reshaping : Some people do not have the facial shape they desire, and in many cases this can be at least partly corrected with dermal fillers, which can give the appearance of higher cheekbones or a stronger chin. The only common side effect of injectable fillers is swelling at the injection sites which can last 1-3 days, and is not something that everyone experiences. You may be aware of the filler at first, but this will pass quickly. As soon as the swelling goes down, the results of your treatment will be apparent. Your wrinkles, folds, and hollows will be significantly reduced. They may even be erased. Your results should last for about six months or more depending on the type of filler used and its placement up- to two years.

Fat Grafting

Fat injections can be an effective way to reduce wrinkles, diminish acne scarring and regain a more youthful appearance. By injecting human fat harvested from your own body, we can enhance facial fullness, fill deep creases, soften facial creases and wrinkles, plump lips and build up shallow contours.

 

Fat injection is a more extensive procedure than the use of other cosmetic injectable fillers because liposuction techniques are involved. These techniques are used to extract the fat prior to injection. Fat injection is either performed alone or in conjunction with a facelift to enhance and soften facelift results.

PRP (Platelet Rich Plasma)

Platelet Rich Plasma (PRP) is a by-product of blood (plasma) that is rich in platelets. Platelet Rich Plasma (PRP) contains various growth factors that can be used to enhance healing and provide a matrix to stimulate collagen deposition for volume enhancement and potential improvement in skin quality and contour. PRP has a very high concentration of growth factors that help stimulate skin rejuvenation and restore volume due to atrophy and aging.

 

During the office procedure a small amount of your own blood is drawn out via the IV. This blood is then placed in the PRP centrifuge machine and spun down. In less than fifteen minutes, the PRP is formed and ready to use to be injected into the skin for rejuvenation after a topical anesthetic is placed on the skin. When PRP is injected into the skin, it accelerates the normal healing process by stimulating collagen formation, revascularization and repair.

 

This process brings long lasting rejuvenation without side effects. Results usually become apparent within 15 days of treatment.

 

Its advantages are that it promotes local tissue growth and repair, stimulates regeneration to eliminate wrinkles, is safe since it is made from one’s own blood and is an office procedure.

 

PRP is recommended for any age groups at different frequencies:

 

Age 30 – 40 years: In this age category the main aim is rejuvenation and prevention of collagen degradation and re- stimulation of new collagen and elastin production, thus attempting to slow down the ageing process. One treatment every 2 years is sufficient.

 

Age 40 – 50 years: In this age category the aim is rejuvenation and to attempt reversal of the ageing process which is more difficult. More re-stimulation of collagen and elastin production is necessary. The initial treatment should be followed by a second treatment 2-6 months later and a third treatment 9 months after the second treatment. Follow-up ‘maintenance’ treatments may be necessary every 12-18 months.

 

Age 30 – 40 years: In this age category the main aim is rejuvenation and prevention of collagen degradation and re- stimulation of new collagen and elastin production, thus attempting to slow down the ageing process. One treatment every 2 years is sufficient.

 

Age 50+ years: In this age category the aim is rejuvenation and to attempt more intense reversal of the ageing process. The initial treatment should be followed by a second treatment 2-6 months later and a third treatment 6 months after the second treatment. Follow-up ‘maintenance’ treatments may be necessary every 12 months

Chemical Skin Peels

Chemical peels have long been used as skin rejuvenation procedures. In the right hands, chemical peels can safely address many common facial concerns, including wrinkles, uneven skin coloration, and a dull complexion. Depending on the severity of the problem the peel is designed to address, you can often see results immediately with little or no recovery necessary.

 

A chemical peel involves the application of medical acids to remove the damaged skin layers in order to produce more youthful appearance. A healing response to the peel can also triggers new collagen formation so the skin appears tighter and more elastic.

 

Chemical peels are most commonly used on the face, but can be used on other body areas such as the chest, back, shoulders and arms to eliminate lines, freckles, or age spots. Patients with more extensive sun damage on the legs can also be treated with chemical peels.

 

Chemical peels have been found to be a powerful complement to other skin treatments, leading to numerous innovations in what is known as combination therapy.

 

Theoretically, chemical peels can be combined with any light, laser, or skin therapy.

 

The most popular combinations include:

These peels are performed in our office and result in exfoliation of the outer layers of your facial skin. The healing time depends on the depth of the peel. As a rule, the deeper the peel, the longer the recovery time. In all cases, avoid sun exposure immediately after a chemical peel and use sun block during the healing process since the new skin is fragile and more susceptible to injury.

 

With superficial peels, you should be able to resume your normal daily activities immediately following your procedure, including the application of makeup and sunscreen. Within minutes to days, depending on the type of peel chosen, you will notice that the treated area is more smooth with less wrinkles, having a more youthful appearance. Your newly rejuvenated skin should have a healthy glow and appear soft and supple. With deeper peels, you will go through a process of shedding and healing that should be complete seven to ten days following your peel. Microdermabrasion

To keep your skin its healthiest, it is best to get periodic deep cleanings. Deep cleanings of the skin involve removing the outer dead layers of skin to expose the fresher, living skin below. This is known as exfoliation and microdermabrasion is an excellent tool for this. Microdermabrasion is a good treatment for acne-prone individuals, those who desire a deep cleaning for healthy skin, and people with dull-looking skin. It removes skin by buffing it with a stream of fine crystals. Air pressure drives the crystals at the skin at a very high velocity, and the same pressure creates a vacuum that draws the crystals away. The process will remove a very thin layer of skin, less than a thousandth of an inch thick. This outer layer consists of dead skin that has lost the luster of living tissue and has suffered damage from the sun and other exposure. It also contains contaminants, pollutants, and dirt that clog pores and damage skin.

 

Microdermabrasion takes less than an hour, and because it is minimally invasive you can go back to work right away with your skin looking healthy and glowing, but not red or irritated.

 

Acne and skin dullness can be reduced with microdermabrasion. Some people see a reduction in skin discoloration, fine wrinkling, and scaly skin after a regular treatment program.

 

Some of the benefits of microdermabrasion are not visible, though. Microdermabrasion prepares your skin to receive topical nutrients, and combining the procedure with a recommended skin care regimen can deepen and lengthen the results you see from microdermabrasion.

Thermage® Skin Tightening

Thermage® is a safe and effective procedure that stimulates your skin to produce more and better collagen, which makes it tighter and healthier-looking. Collagen is the major building block of the skin. This protein helps the skin to be tight and elastic, meaning that it returns to its shape after pulled or otherwise deformed. Collagen-rich skin resists wrinkling, is thicker, and appears more youthful. Thermage® uses a unique form of radio frequency (RF) energy to simultaneously tighten existing collagen and stimulate the skin to produce more collagen. The result is tightened skin producing a kind of “lift” without a single incision. With minimal treatment discomfort and no recovery, tighter, smoother skin, an overall more youthful appearance can be achieved. The energy is delivered through a hand-piece directly applied to the skin heating the deeper layers causing tightness and simultaneously cooling the skin for comfort. The addition of the new CPT hand-piece allows the heat to be more evenly distributed across the treatment area, permitting the Thermage® CPT system to heat four times more tissue to therapeutic temperatures than rival technologies and thus significantly increasing its effectiveness.

 

Thermage® is effective on:

After treatment the skin may be slightly red, but most people resume regular activities immediately after treatment. There is no recovery time or special follow-up care. The results may appear immediately but usually, measurable tightening and contouring appear gradually in the 2 to 6 months post treatment. Results can last years depending on the skin's condition and ageing process.

 

While Thermage® cannot produce the same level of effects as a surgical facelift, it often produces visible results that for many people are well worth the tradeoff of a non-surgical procedure with no recovery.
